Your first visit to City Point Dental
We recommend arriving 10–15 minutes early if you need to complete any paperwork before your appointment. To make check-in quicker and easier, you can complete your new patient forms at home using the fillable PDF or print them and fill them out by hand. Your forms will be sent to you electronically once your appointment is confirmed.
During your appointment
Your first visit is all about getting to know you. Most new patient appointments last approximately 60–90 minutes, giving our team time to understand your concerns, discuss your goals, and thoroughly evaluate your oral health. During your appointment, we’ll perform a comprehensive tooth-by-tooth exam, take necessary digital X-rays, complete a head and neck oral cancer screening, and create a personalized treatment plan. A professional cleaning may also be completed when clinically appropriate. We’ll show you what we see, explain your options in plain language, and answer your questions without judgment or pressure.
To help your visit go smoothly, please bring:
- Your dental insurance card, if applicable
- A list of your current medications, supplements, and medical conditions
- Any recent X-rays or dental records from your previous dentist
- Completed new patient forms, if you filled them out beforehand
- Any questions or concerns you’d like to discuss
- Your favorite dental joke—yes, really
